Good Faith Estimate Notice
Your Right to Receive a Good Faith Estimate Under the law, health care providers are required to provide clients who are not using insurance with an estimate of expected charges for medical and mental health services.
You have the right to receive a Good Faith Estimate that explains the anticipated costs of counseling services before you receive treatment.
Counseling Fees:
The following are estimated fees for services provided by this practice:

No Call No Show (No cancellation before 24 hours prior to session) $150
Initial Assessment (60 minutes) $150
Individual Counseling Session (53–60 minutes) $125
Individual Counseling Session (30 minutes) $75
Family Counseling Session (60 minutes) $150
Estimated Cost of Treatment:
The total cost of counseling services depends on your individual treatment needs and goals. Actual treatment length cannot be guaranteed because counseling is individualized.
For example:
12 weekly individual counseling sessions at $125 per session: Estimated total cost: $1,500
This estimate is not a contract and does not obligate you to obtain services from this practice. Actual costs may vary based on the number of sessions attended, changes in treatment needs, or additional services requested.
Questions or Concerns:
You may request a Good Faith Estimate at any time before scheduling services or during treatment.If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
